I flew Medellin-Fort Lauderdale in Nov 2012. At least 50% of the passengers did not speak English, as they were tourists, elders, etc.
I noticed the crew was making all their safety announcements in English. When I asked a flight attendant why, she said, "Because it's not mandatory, and they would have to pay a bilingual attendant more. The only bilingual flight is from Haiti, because the Haitian government requires someone in the crew to speak Creole/French by law."
I was so glad nothing went wrong on the flight, otherwise how on earth would the crew give instructions to the passengers? How on earth do they get away with this?
